Poppy Homecare will always strive to give our clients the best possible service whilst at the same time being committed to allowing our clients the opportunity to be independent and do as much for themselves as they can.

Our focus and core competence is with adults with learning disabilities, physical disabilties and autism.  All packages will be built around the needs, hopes and safety of our clients.

 

The client and their family or advocate are central to the care plan and only once we have taken advice and listened to all involved will we make an assessment of the care package required by each individual.

 

We offer a wide range of individually tailored care packages compiled specifically for our clients with varying levels of wants and needs in mind.

 

Support offered can be for day to day living tasks, such as shopping or going to the cinema or for more complex needs, where perhaps there is specialist input required.  Each package is aimed for the client to reach his or her full potential, regardless of any barriers they may face.

 

Transition:

Poppy Homecare recognise that many clients find transition difficult to cope with, whether that’s going from one service to another or the daunting prospect, for parents and children of leaving school and entering the world of adult services.  That’s why we offer a robust transition package designed to make the transition to a new service an enjoyable and relaxed process for all concerned.

 

Direct Payments:

Direct payments or self-directed care is a procedure whereby a service user or their advocate may get direct payments to buy services they require themselves, rather than relying on the local authority to provide services.  This can be especially useful if you want to build your own package and use components from various service providers which best suit your needs.

 

Our services include but are not limited to:

 

Practical Care

 

  • Household chores

  • Support for outings and holidays

  • Help with accessing the community

  • Domestic tasks, such as cooking a meal

  • Escorts for college or work placements

  • Trips to the theatre, concerts or cinema

  • Advice and support on active and healthy choices

  • Coping with transition

  • Direct payments or self-directed care advice

 

Personal Care

 

  • Help with bath and Showering

  • Medication Assistance

  • Dressing/undressing

  • Night care

Holidays

  • We can also source and arrange short breaks and holidays including carers both in the UK and abroad.  Holidays are based on the individuals needs and interests.
